About the Course:
The initial certification class lasts approximately eight hours, plus a one hour exam.

The four hour re-certification class is designed to renew a three-year Minnesota Department of Health Food Manager Certificate (testing is not required,) or a five-year ServSafe® Certificate (testing at the re-certification class required.) You must hold a current MDH FM Certificate or have a valid ServSafe® Certificate that has not expired to be eligible to take this class. If you are unsure if you are eligible, please call the Minnesota Department of Health at (651) 201-4500.

About the Test:
A minimum score of 75% is needed to pass. Tests are electronically scored and the results are received in approximately two weeks. A certificate of completion and the MDH application form is sent to each participant with the test scores. It is the responsibility of each person to apply for the Minnesota Department of Health Certification. The State filing fee is $35.

You can register online or download and complete the form at the bottom of this page and mail, fax or e-mail it to the the addresses listed on the registration form. To register by phone, call Mike Pantaleo at (763) 785-7307. Please remember to specify the course you would like to register for. Registration deadline is 10 days prior to class.

Initial Certification Cost:
Sysco Minnesota Customers: $145 per person
Sysco Club Members: $135 per person
Non-Customers: $170 per person
Course book and testing fee are included. Sysco will also provide a continental breakfast and lunch. Information about the course and book will be sent out approximately 2-3 weeks prior to class date. Late registrants (within one week of class date) will receive book the day of the class.

Re-Certification Cost:
Sysco Minnesota Customers: $65 per person
Sysco Club Members: $55 per person
Non-Customers: $75 per person
New book w/exam sheet: $70 each
New book w/o exam sheet: $55 each
(Books not required for re-certification class)

Payment:
Payment must accompany registration if the business is not a Sysco customer, or if customer has cash or C.O.D. terms. The course fee can be billed to active Sysco customers. No refunds once book is mailed. Rescheduling is subject to a $30 fee and requires three days notice. All no-shows will be billed. Credit cards are not accepted.
